In consideration of being permitted to participate in the swimming lessons offered by Sunshine Swimming, LLC, I, the undersigned, do hereby agree to the following:
1. Inherent Risks: I understand and acknowledge that swimming inherently involves risks of drowning, injury, or illness. While Sunshine Swimming takes all reasonable precautions to ensure the safety of participants, these risks cannot be entirely eliminated.
2. Release of Liability: On behalf of myself and my minor child(ren) listed below, I hereby release and forever discharge Sunshine Swimming, its instructors, owners, agents, employees, and representatives from any and all claims, liabilities, or damages arising out of or in any way connected with participation in swimming lessons, including but not limited to claims of negligence or breach of duty.
3. Medical Disclosure: I certify that my child(ren) listed below are in good health and have no physical or medical conditions that would limit their participation in swimming lessons. I further agree to disclose any changes in my child(ren)’s health conditions that may affect their safety during lessons.
4. Assumption of Risk: I voluntarily assume all risks associated with participation in swimming lessons at Sunshine Swimming.
5. Emergency Medical Treatment: I authorize Sunshine Swimming to seek emergency medical treatment for my child(ren) in case of an accident or injury, and I agree to be responsible for all associated costs.
Multimedia Release:
I grant Sunshine Swimming LLC the unlimited right to use and/or reproduce photographs, recorded video, and/or the voice of me, my family and child(ren), in any legal manner and for the internal or external promotional and informational activities of Sunshine SwimmingLLC, including but not limited to print, publicity, advertising, and web content. I further understand that by signing this release, I waive any and all present or future compensation rights to the use of the above-stated material(s).

Electronic Signature
The Liability release, assumption of risk and policies is an electronic contract that governs your use of and access to the Sunshine Swimming website and services. Upon agreeing to the Liability release, assumption of risk and policies when completing the swim lessons registration form, you acknowledge that you understand and agree to be legally bound by the terms, conditions, and notices contained or referenced herein. Checking the box or using a digital signature creates an electronic signature that has the same legal force and effect as a handwritten signature.
